JPS Health Network Opens Fort Worth Medical Home

JPS Health Network, a healthcare delivery provider based in Fort Worth, opened its JPS Medical Home at True Worth Place on Monday. The space is designed to improve population health and healthcare delivery for the homeless or those who cannot afford care in Fort Worth, according to JPS.

JPS’s 12,000-square-foot, in-house clinic at True Worth Place—Fort Worth’s new resource center for the homeless—will provide primary care services along with dental and behavioral health services. The clinic also includes a radiology clinic, lab, clinical pharmacy, and a case management unit, which allows patients to have a “one-stop shop” for required health services.

JPS’ Care Connections team, an initiative aimed at providing street medicine, will have its headquarters at the new facility. Physician assistant Joel Hunt will treat patients at the clinic along with other medical providers.
