DFW will see its first Oak Street Health clinic this summer, as the value-based model expands its nationwide network. The centers deliver care to Medicare patients and are expanding to Texas and Tennessee this year.

“We are deeply committed to providing integrated, preventive care that helps seniors live their best lives, and our planned growth for early 2020 reflects this commitment,” said Tamara Jurgenson, Chief Growth Officer of Oak Street Health via release. “Our community-based approach makes a meaningful difference in our patients’ health outcomes, and we strive to reach as many people as we can. We’re excited for our care teams to become pillars in these new communities, and for Oak Street Health to make a positive economic impact as we have in our existing markets across the country.”

Oak Street Health’s value-based model emphasizes increased time with providers and individualized treatment plans, 24/7 patient support line, and access to transportation to and from appointments. Behavioral health and Medicare instruction are also part of their program.

The network was founded in 2012, and now has 50 centers in Illinois, Michigan, Indiana, Pennsylvania, Ohio, Rhode Island and North Carolina, and its patients have experienced a 41 percent reduction in hospital visits and a 49 percent reduction in emergency room visits compared to Medicare benchmarks.

“When we set out to rebuild healthcare as it should be in 2012, we wanted to create a unique model that put the patient first,” said Mike Pykosz, CEO of Oak Street Health via release. “We’re proud of what our teams have already accomplished to improve healthcare quality and experience for our patients and communities. Our innovative approach works, and we’re delighted to continue expanding to bring the Oak Street Health care model to more people across the country.”
