Best Doctors in Collin County Nomination Form

Please use this ballot to recommend up to three of your peers in each specialty. Only doctors practicing within our geographic boundaries (here) with a valid medical license are eligible to nominate, and self-nominations will be disregarded. Please do not nominate a doctor for more than one category. Doing so can result in removal of ballot. Please make sure all nominees are located within the boundaries of Collin County, and are currently accepting patients.

Our form auto-populates using our online directory. If a name does not show up, it is not yet in the directory. Click the option to “write-in”, and note that write-in nominations count as much as auto-populated ones.
