DFW will see its first Oak Street Health clinic this summer, as the value-based model expands its nationwide network. The centers deliver care to Medicare patients and are expanding to Texas and Tennessee this year.

Oak Street Health’s value-based model emphasizes increased time with providers and individualized treatment plans, 24/7 patient support line, and access to transportation to and from appointments. Behavioral health and Medicare instruction are also part of their program.
The network was founded in 2012, and now has 50 centers in Illinois, Michigan, Indiana, Pennsylvania, Ohio, Rhode Island and North Carolina, and its patients have experienced a 41 percent reduction in hospital visits and a 49 percent reduction in emergency room visits compared to Medicare benchmarks.
