StratiFi Health Doubles Up URAC Accreditations

StratiFi Health is just the second health care company in the country to earn a URAC accreditation for Transitions of Care, while adding a Case Management accreditation from URAC as well.

“We are honored to receive the URAC accreditations and proud of our team’s dedication,” said Christopher Crow, M.D., CEO of StratiFi Health via release. “It is a reflection of our commitment to helping communities thrive and a continuation of our dedication to being health care innovators.”

URAC is an independent accreditation, certification, and measurement nonprofit that uses evidence-based measures to improve the quality of healthcare. StratiFi Health is a physician practice services and population health company that hopes to allow practitioners to stay independent and care for their patients.

“StratiFi Health earned the URAC accreditation, in part, because it helps physicians effectively integrate case managers and care teams into the primary care that patients receive,” said URAC President and CEO Dr. Shawn Griffin via release.
