Encompass, Dallas-based Medicare Home Health Provider, Partners With ACO

Premier Patient Health Care and Encompass Home Health & Hospice announced they will partner, with Encompass bringing post-acute care to Premier’s ACO network.

The collaboration aims to improve time and cost efficiency for patients and providers alike. By easing and increasing communication between physicians, hospitals, and other providers, Premier PHC and Encompass plan to reduce inefficiencies in care coordination and transitions for ACO patients. Other benefits include fewer unnecessary medical tests and forms and more centralized customer service support.

“Attention to cost control and patient care have never been more important, said Anwar Kazi, COO of Patient Healthcare. “According to the Center for Medicare and Medicaid Services, U.S. health expenditures are expected to reach approximately 20 percent of GDP by 2021.”

Along with improved cost-efficiency, patients will see better quality healthcare, “with statistics showing that coordinated care participants enjoy improved health, greater autonomy, and fewer hospital admissions when compared to patients without access to coordinated care,” according to a release.

Premier PHC currently serves more than 20,000 patients in five regions in North Texas and Southern Oklahoma. Specifically, Encompass vows to improve access to post-acute care for ACO patients. The company, a top 10 nationally ranked provider and the largest Medicare-certified home health provider in Texas and Oklahoma, specializes in home and hospice care and care transitions.

Other promised benefits of the partnership include streamlined referral, better care management post hospital discharge, completion of Health Risk Assessments, and the coordination of follow up appointments with primary care physicians and specialists. Based in Dallas, Encompass currently serves over 50,000 patients.

Medicare benefits such as cost, coverage, and self-selection of providers will remain the same for patients treated in the Premier PHC ACO.
